The Elecom Deft Pro has been good for me.
I come from the Logitech Trackman camp, but it's pretty similar to the Microsoft trackball.
Is this the one you have?
If so, I have no objection to using USB - and it sounds like that would solve the majority of the problems.
I'm married to a Canadian, not sure why that your your nationality matters.
I highly recommend the Elecom Deft Pro. It is the modern version of the style of trackball linked in your ebay listing. They have the Huge as well, but I feel like that's a bit too large for me.
I can work with thumb balls, but that slim blad thing looks uncomfortable to me.

So this is really weird. I was eventually able to pull out the Deft Pro's bluetooth device descriptor with wireshark, and this is what I got:

To answer your other questions: There's only one model of this mouse that I know of. It has wired, wireless (rf), and wireless (bt) modes based on various switches on the device. Amazon japan does have a variant listed as M-DPT1MR*X*BK, but the specs are the same and for some reason it's listed as an Amazonbasics product. I haven't seen one in the wild, so I don't know if there's any difference or just a listing error.
Finally: I should be around this weekend, so I should be able to give your driver a test. Just send me a link/instructions/whatever when you have it ready.

The trackball can do wired, 2.4 Ghz wireless with a dongle or Bluetooth.
If you're willing to do some modding, Elecom's Deft Pro can be turned into a great trackball. You'll want to replace the stock ruby bearings with 2.5mm G5 Si3N4 bearings and put some dielectric grease on the scrollwheel or remove the spring to have it free wheel. Here's a great guide on how to take the Deft Pro apart. You'll need to be careful with the two ribbon connectors and you'll have to use some force to free the ball socket from the base. I had a 20 degree angled wrist rest 3D printed through Treatstock and I also use Elecom's Fittio small wrist rest.
The ball is so much smoother than stock and it doesn't bind like it did even after I upgraded to ceramic bearings. I have three of the modded Deft Pros now and IMO it's the best trackball available. It has the most buttons of any trackball mouse currently available and you can remap them using Elecom's Mouse Assistant software.
Elecom's quality isn't always the best, but the Deft Pro has a better build than the Huge and the warranty difference backs that up (six months for the Huge, three years for the Deft Pro). Obviously opening the trackball voids the warranty, but it has high-qualilty Omron switches that could be replaced if you know how to solder.

Have you looked at the Deft Pro? It looks like it has a similar shape to the T-RB22, albeit with a different button layout. While I haven't used the Huge, its shape makes your hand sit flat rather than at an angle like you get with the T-RB22 and Deft Pro. It also looks like it doesn't have enough palm support which might be part of the problem. The Deft Pro fills my hand nicely, even though I have fairly long fingers.
The biggest problem with the Deft Pro is the included artificial ruby bearings. I've been meaning to swap mine out for ceramic bearings but hadn't taken the plunge yet. Ended up getting these bearings but have yet to swap them out. My other issue is Elecom's horrible MouseAssistant software. It will randomly stop working and start acting like there is an invisible window above whatever program I'm using when I left click. I also remapped the right click and it will have random problems as well. Restarting MouseAssistant fixes the problem, but it's frustrating since their software is the only way you can remap the non-standard buttons. I'm willing to deal with those problems since I like the Deft Pro so much.
While I don't think this will solve your problem, I just got an angled wedge for my Deft Pro and it helped with the arm and shoulder pain I was experiencing. Someone made a wedge for the Huge as well if you want to see whether that helps.
I would like to get a GameBall for gaming at home (Deft Pro is at work), but the shape and cost are what's holding me back. I haven't seen any reports of knuckle pain with the GameBall, though I haven't looked into it either. The MX Ergo's shape is similar to the Deft Pro, and with a 3D printed wedge you can get the tilt feature with a finger trackball. Thumb trackballs are a no-go for me as well, since my ergo journey started with pain between my hand and thumb. Ploopy's Classic Trackball is another option, but $175 for a product with multiple 3D printed parts is too much.

I bought it last year, so it must be the updated version.
I currently alternate between an Apple Magic Trackpad 2 (it works in Windows with this driver) and an Elecom Deft Pro. I like the gestures on the trackpad but the trackball is probably more comfortable because it allows you to rest your whole hand rather than having to strain it by hovering constantly.

I have large hands too. Length wise, my right hand is 8 inches with a 4.5 inch palm. I just got both the Deft Pro and Huge for comparison purposes. While the Huge is definitely bigger in size technically, there's more "mouse" for your palm on the Deft Pro. I do like the size of the trackball so far today on the Huge, but I am starting to see that the wrist rest ultimately makes the mouse dangerous to use for large hands... maybe even medium hands. I can tell a "twinge" of sorts start on the right side of my wrist. I would stop using it if you're feeling anything like I am... and I just got it today too.
Yea, there's also buttons on the left side "fn1" etc. I am a little confused with the models.

The Elecom pages you linked are absolutely identical. I looked them both up on Amazon and the specs on those two pages are identical as well.
Maybe someone else knows for sure, but I'm willing to bet that the MRBK is an outdated SKU. It's more lightweight than all of the gaming mice, since you're just moving a ball around.
